Overview

Health Coach role within the Te Tumu Waiora collective, supporting primary health care teams across three GP practices in North Canterbury.

You will work with tangata whaiora experiencing mild to moderate mental health distress and related physical health issues to support behaviour change, self-management and wellbeing through lifestyle changes, motivation and connection.

Responsibilities

Work across 3 GP practices as part of the Te Tumu Waiora collective.

Support tangata whaiora to improve mental health and wellbeing and make positive lifestyle changes.

Assist with behaviour change and self-management to improve wellbeing through lifestyle change, motivation and connection.

Qualifications and Experience

Minimum of level 4 Health and Wellbeing, or a recognised equivalent.

A full driver

0en license.

Confidence working independently and as part of a multidisciplinary team.

Be flexible and proactive.

Knowledge of holistic health and behavioural change models is preferred but not required.

Experience in a primary health, physical health or mental health field is preferred.

Role Details

Working 32 hours per week, 8 : 00 am – 4 : 30 pm, days to be confirmed, based in North Canterbury.
