We are looking for a highly motivated person to lead our Needs Assessment and Service Coordination and Disability Support Services team.
This role is an exciting opportunity to provide leadership and management for the FOCUS team, and to work closely with people with disability and older adults to extend manaakitanga and support them to live good lives with their whanau and community.
This is a permanent full-time position working Monday to Friday 80 hours per fortnight (1.0 FTE).
The role is based in Masterton.
About the Team / Service / Location
The FOCUS team undertakes assessment and service coordination in the inpatient setting at Wairarapa Hospital, and with clients in their own homes and residential care settings across the Wairarapa region.Learn more about living in the Wairarapa region click here Wairarapa
About you
You will be a registered Health Professional with at least three years' experience within an older persons' health and / or disability setting.
You will ideally have leadership experience, and will have a sound knowledge of NASC and Disability Support Services function.
You will have a strong client centred approach, superb communication skills and the ability to form strong collaborative relationships with stakeholders across a range of settings.
