As a community trust we are governed by a Board of Trustees with representation from the health workforce, Maori health providers, iwi, and the local community.

At WCH we are committed to growing and supporting primary care in our communities, providing top quality healthcare services, and working to eliminate health inequities.We're looking for a Health Pathways Coordinator to support clinicians and health services across the Coast by localising, reviewing, and promoting Health Pathways.

This is a unique opportunity to contribute to better healthcare experiences for people and whanau while working in a supportive, community-focused organisation.About the roleAs our Health Pathways Coordinator, you will : Work with clinical leaders to localise and update West Coast Health Pathways.Liaise with Health Pathways reviewers and Health NZ / Te Whatu Ora Waitaha colleagues.Provide training and support to clinicians using Health Pathways.Promote Health Pathways across general practice and health services.Contribute to continuous quality improvement across the PHO.About youYou will bring : A strong understanding of health equity and responsibilities under Te Tiriti o Waitangi.Knowledge or experience in the health sector.Excellent written communication and attention to detail.Strong self-management and IT skills (Microsoft Office essential).
