In this pivotal role, you will lead the delivery of proactive, evidence-based care for people with Long Term Conditions, while supporting General Practices across the region.
You will act as a key connector between practices, THINK Hauora, and other healthcare providers – ensuring seamless, coordinated care.
This role also provides expert guidance on clinical, contractual, and funding matters, and involves regular local travel from either our Palmerston North or Horowhenua office.You will be responsible for : Leading a multidisciplinary team to deliver coordinated, high-quality care for patients with long-term conditionsActing as a key support and liaison for general practices, providing clinical guidance and connecting them with THINK Hauora servicesStrengthening partnerships with general practices by coordinating patient care, facilitating referrals, and ensuring smooth communication and continuity across servicesProviding financial and contractual support to General Practices to ensure compliance and optimise fundingLeading and supporting the team to deliver high-quality, compliant care through evaluating care plans, monitoring performance, conducting audits, and promoting continuous learningYou will have : Bachelor's degree in nursing, with a current practicing certificate.
A Prescribing qualification is preferred.Minimum 10 Years clinical nursing experience in Primary Health, with Practice Management experienceProven ability to lead and manage a multidisciplinary team.Strong leadership skills with experience motivating teams and providing clinical guidanceHighlights of Working with UsAt THINK Hauora you will work alongside a diverse team of professionals who are passionate about delivering primary health care to our region and making a positive impact to our communities.
