He Waka Tapu is a Kaupapa Māori organisation who have been supporting individuals and the community for 30 years. He Waka Tapu works to increase the health and wellbeing of Whānau.
About the role
Te Tumu Waiora – te reo for ‘to head towards wellness’ – is a model of support for patients that ties together physical, mental, and social needs.
The model puts mental health and wellbeing at the heart of general practice with roles of Health Improvement Practitioners, Health Coaches and Community Support Workers, working as part of the general practice team. The Community Support Worker links patients to services throughout the community.
The Community Support Worker role aims to connect directly with patients to assist them to make any necessary changes to improve their health and wellbeing and work towards an achievable wellness. The role involves being an integral part of the general practice team, working directly with individuals and their families to help improve their mental health and wellbeing. The key priorities of the role include :
We are hiring to fill a couple of roles and have permanent full-time and part-time mahi available. You may be required to work in a number of settings across Canterbury in general practice, the community and within our Kaupapa organisation.
