About the Role
Supportlinks is the Needs Assessment and Service Coordination (NASC) service for the Te Pae Hauora oRuahine o Tararua MidCentral district. We connect people to the support and services they need to livewell and independently — whether they're experiencing acute or short-term needs, living with long-termhealth conditions, or are older people / kaumātua with age-related health conditions.As part of the OPAL Community Services team, you'll work across the MidCentral District alongside adedicated and supportive transdisciplinary team — including geriatricians, occupational therapists,physiotherapists, social workers, Māori connectors, and allied health assistants. Together, you'll make agenuine difference in the lives of those we serve.We're offering a permanent part-time role based in Palmerston North for someone who is enthusiastic,energetic, resilient, and has a strong “can-do” attitude.If you hold a current Annual Practising Certificate in Nursing, Occupational Therapy, Physiotherapy, orSocial Work, this is your chance to join a supportive, values-driven team and make a real differenceevery day.
