Home Health Lpn - Raynham, Ma

Caretenders is hiring for a full-time LPN to join our passionate team in Raynham, MA!

Coverage areas will include : Taunton, Raynham, and surrounding towns.

The Licensed Practical Nurse in Home Health is responsible for providing quality patient care and performing technical skilled care in the patient's home.

Responsibilities

Provide high-quality clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care, and in coordination with other healthcare team members, following all infection control procedures.

Do not alter the plan of care.

Observe and report the patient's nursing needs to the supervising RN.

Communicate effectively with physicians, patients, family members, and agency staff regarding patient care, visit schedules, and any changes, ensuring documentation and coordination of care.

Document patient visits thoroughly, maintain accurate medication lists, and ensure timely documentation per policy.

Participate in performance improvement initiatives aimed at reducing hospitalizations and improving patient outcomes.

Attend all meetings, including case conferences, punctually, and participate actively, including having the camera on for video meetings.

Maintain a positive attitude, assist patients and the agency, and be open to learning new skills.

Adhere to and participate in the agency's Episode Management process.

Perform all other duties as assigned.

Education and Experience

License Requirements

Current LPN licensure in the state of practice.

Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

CPR certification.
