  • Permanent, part-time role (0.6 FT, 24 hours per week)

    About the role

    Our Needs Assessment team is passionate about the community it serves and works effectively, keeping our clients best interests at the top of our minds.

    In this role, you will :

    • be joining a hardworking team and will play an important part in its success
    • assess people who are predominantly over the age of 65 and need support because of aging
    • assess clients in their homes and in a variety of health settings for example hospital, hospice, aged care facilities
    • utilise the national assessment tool interRAI to assess and determine best outcomes for our client group.
    • The assessment is a structured, comprehensive electronic tool that you will work through with the client in a conversational manner

    • write comprehensive reports in digital systems
    • create a plan of care that is client centred, and goal focused including developing support packages in a holistic and culturally appropriate manner for older people who require long term support in the community
    • play a key role in connecting people and their whanau with formal and informal support services to assist them to achieve their goals.

      About you

      You will :

    • be a registered health professional with a current practising certificate
    • have experience working with older people in either a hospital, community, or aged care environment
    • need an excellent knowledge of community resources and services that are available to the older person
    • have a current NZ driver's licence (essential)
    • be competent with computers and be able to perform the duties listed above to a high level.
    • Salary range

      This role is covered by the terms and conditions of the Allied, Public Health, Scientific and Technical CEA and the salary will be in accordance with the Pay Scale, between Step 1 $77,087 and Step 8 $110,210 p.a pro-rata and the NZ Nurses Organisation Nursing and Midwifery CEA 31 March

    • CEA and the salary will be in accordance with the Pay Scale, between Step 1 $75,773 and Step 7 $106,739 p.a pro-rata dependent on relevant experience.
