About the role :

We are on the lookout to find an experienced Tapuhi Hapori (Community Nurse) to join our Early Years Health Services team.

The Tapuhi Hapori (Community Nurse) is responsible for the provision of primary health care services to whanau in the community. This role will work across our Early Years Health services.

The Community Nurse is responsible for establishing and maintaining a therapeutic partnership with patients and their whaanau to achieve optimum health and well-being through appropriate assessment, critical analysis of clinical information, clinical reasoning, care planning, intervention, evaluation and education.

Flexibility is key as this role also involves advocating for whaanau and working across diverse settings, including clinics, homes, and community spaces, to maximize health and well-being outcomes.

This role is based in Huntly, Waikato. This a full-time role from Monday to Friday from 8 : 30am to 5pm.

Responsibilities include but are not limited to :

Effectively establishes and maintains therapeutic relationships with patients receiving health care and their whaanau

Evaluates responses to nursing interventions with the patient, their whaanau, and the health care team

Anticipates work flow each duty and organises resources and interventions accordingly, displaying a sound understanding of emergency procedures, plans and lines of communication

Ensures any situations that may compromise the safety and wellbeing of patients and their whanau are quickly identified and addressed

Accesses, understands, and utilises relevant policies and procedural guidelines that have implications for nursing practice, and identifies new insights and / or potential for improvement and change

Provides direction and appropriate delegation to nurses and other care or support workers within the clinical environment; monitoring and evaluating the care provided within the centre

Utilises relevant research findings, and person-centred evidence-based knowledge to underpin nursing activities and patient care

Participates in supporting, developing, and teaching colleagues to enhance professional nursing care outcomes

Supports and promotes actions and initiatives that enable risks to be identified and eliminated or reduced with particular focus on any risks which have high cost or significant safety implications

Ensure any complaints are appropriately responded to in line with processes and policies.

To be considered for this role ideally you will need to have :

Registered Nurse with current Annual Practising Certificate

Primary Health Experience – In GP Practice or Community setting

CPR Core Skills Certificate

To attend all Core Competency trainings as required by the organisation

Experience in a similar role would be an advantage or able to demonstrate an understanding of NZ Health especially in relation to Rangatahi, Mental Health and addictions services and advocating for Pepi, Rangatahi and whanau.

New Zealand registered Occupational Therapist with a current practicing certificate

Be legally entitled to work in New Zealand / Aotearoa (Citizenship, Work Visa or NZ Residency)
