    The Role

We are looking for a dynamic Primary Health Network Team Lead to oversee the daily operations of our multidisciplinary team. • Strengthening partnerships with general practices by coordinating patient care, facilitating referrals, and ensuring smooth communication and continuity across services
    • Providing financial and contractual support to General Practices to ensure compliance and optimise funding
    • Leading and supporting the team to deliver high-quality, compliant care through evaluating care plans, monitoring performance, conducting audits, and promoting continuous learning
    • You will have :

    • Bachelor’s degree in nursing, with a current practicing certificate. A Prescribing qualification is preferred.
    • Minimum 10 Years clinical nursing experience in Primary Health, with Practice Management experience
    • Proven ability to lead and manage a multidisciplinary team.
    • Strong leadership skills with experience motivating teams and providing clinical guidance
    • Highlights of Working with Us

At THINK Hauora you will work alongside a diverse team of professionals who are passionate about delivering primary health care to our region and making a positive impact to our communities.
