ProCare Fresh Minds has an exciting opportunity for someone wanting to temp on a contract until the 5th of July 2026, 0.7 FTE as a Health Coach in Clendon, South of Auckland .
We are inviting expressions of interest from candidates who are keen and passionate about supporting people within their community. Whether you are looking to build a career or are already a trained / certified Health Coach , we’d love to hear from you!
You would be based at the heart of primary care in a GP Practice in Clendon, South of Auckland, where your role is all about empowering people to reach their own health and wellbeing goals. This diverse and fulfilling new role empowers whaiora and whānau to gain the knowledge, skills, and self‑confidence to achieve their own health and wellbeing goals.
This includes lifestyle changes such as smoking cessation and exercise, promoting self‑management especially for long‑term conditions like diabetes, and helping navigate primary care and the wider social services they need to succeed.
About you
This role may suit someone who is a certified Health Coach (Te Pou trained) or has been a peer support worker, volunteer, and has lived experience supporting a whānau member with a long‑term condition such as diabetes.
Working as a Health Coach offers a busy but incredibly rewarding environment, where you will tangibly see the difference and positive impacts you can have on people’s lives. Full specialist training and support will be provided for all successful applicants.
