The Day Hospital is an award winningservice that uses an extended scope of practise framework for holistic healthcare management.

The service specialises in management of falls, stroke rehabilitation andParkinson's disease as well health preventionstrategiesandtreats a wide range ofmultipathologies affecting the older person.We aim to support individuals to maintain independence and manage their long term health conditions effectively, reducing hospital admissions and helping to facilitate early supported discharge of patients from hospital with our outreach service, ambulatory care team and collaboration with the acute older persons Hospital at Home service

Main duties of the job

This service offers a range of interventions in both an outpatient setting and within the patient's home environment.

As a band 6 nurse you will play a vital role in the delivery of high quality patient centred care to older people with a variety of complex health and social care needs.

You will work collaboratively within a multidisciplinary team.

This role offers opportunity to develop skills in holistic assessment, care planning and the delivery of a range of treatments and interventions across different settings across Christchurch and Poole Hospitals and within the community.

A full drivers licence and access to a car for work purposes is essential for the role.

This role requires an active UK NMC registration

Base Location : XCH DH

Interview Date : 2 / 12 / 2025

Person Specification

Qualifications

Nursing Degree or equivalent qualification

Evidence of CPD

Post Graduate Education

Ability to assess, manage & treat patients with complex multi pathologies at highly specialist level

Skills in goal setting & using outcome measures

Knowledge and skills in discharge planning and onward referral

Skills in holistic assessment

Ability to produce clear written documentation of intervention & reports

Venepuncture and cannulation

ECG

IV Medication

Therapeutic moving & handling skills

Ability to prioritise clinical workload

Male catherisation

CVAD training

Ability to teach & supervise Junior staff & assistant

Experience

Experience of Elderly rehabilitation within the acute and community settings.

Experience of facilitation of the hospital discharge process

Work allocation / caseload management

Supervision of more junior staff

Recognition of deteriorating patient

Minimum of 18 months experience including an acute NHS setting

Involvement in change and service development

Knowledge

Understanding of the pathologies that elderly patients commonly present with

Therapeutic intervention for patients with chronic, multi pathology and complex conditions

Understanding of NSF's, Clinical Governance and their relevance to clinical situations

Good knowledge of long term conditions, Dementia, Falls, Fractures, Osteoporosis and infections commonly affecting the older person

Understanding of NSF's Clinical Governance for Older People, Falls, Parkinson's and Intermediate Care

Understanding of the interface between hospital, intermediate care and community services

A sound understanding of capacity issues and ability to recognise need for formal assessment

Other Requirements specific to role

Ability to use a keyboard to access and input information from a computer based system

Ability to move and handle patients and equipment and work in confined spaces

Ability to participate in extended hours and weekend service

Ability to drive, hold a drivers licence valid in the UK and have access to a car for work purposes

Ability to deal appropriately with difficult and sensitive situations

Ability to teach patients, relatives, carers & staff

Ability to work effectively in a team

Experience liaising with outside agencies and disciplines Well motivated

Good time management

Ability to work under pressure
