Service Coordinator - Manawatu / Horowhenua

This role coordinates services provided by CCS Disability Action. You will work in partnership with individuals and their families to develop, deliver and / or coordinate a range of person–directed services that aim to empower and enable the individual to achieve their personal lifestyle goals.
