Job Title : Community Nurse – Whānau Health & Home Support Assessments

Location : Whaioranga Trust - 531 Welcome Bay Road, Tauranga

Reports To : Manager

Hours : 30 hours per week

Remuneration : $36-$45 per hour (dependant on experience), Support with Further Study!

Employment Type : Part-time Permanent

Purpose of the Role

To provide culturally responsive nursing through the assessment of home-based support referrals for age-related disabilities and the delivery of community-based health checks. This role prioritises equitable outcomes for whānau Māori and ensures that services are delivered in a manner that upholds mana, fosters whanaungatanga, and supports holistic wellbeing.

Key Responsibilities

• Home-Based Support Assessment – completing clinical assessments in disabilities sector to assess for home based support services
• Community-Based Health Checks – conduct health and wellbeing assessments for whanau, screen for long-term conditions, actively target and engage with whānau Māori, ensuring an increase in accessibility
• Clinical Guidance & Support – provide clinical guidance to teams, ensuring practice aligns with clinical and cultural safety, strengthen quality improvement and care delivery
