About this role

To provide support and assistance for people with mental illness living in the community, encouraging them to build on their strengths and the strengths in their community, encouraging their needs for developing skills, accessing community resources, developing functional relationships and independence.

What you’ll be doing

• Working with a defined caseload of individuals to enhance their community living skills and to develop self‑management of their lives
• Assisting clients to make connections with appropriate services / agencies and ensure required services are received
• Active participation in shared planning with the client, their whanau / family, other clinical and non‑clinical providers
• Communication and liaison with health and community agencies
• Maintaining accurate health records and statistical data

About you

Eligibility

Applicants for this position must have the legal right to work in New Zealand and consent to undergo a criminal record check process.
