Job descriptionProvide rehabilitation and support to individuals within their community Work within a multi-disciplinary team to help clients achieve their rehab goals Knowledge of community services pathways Skilled in assessing and treating concussion and mild to moderate traumatic brain injury Experience in assessing and treating cervical spine, vestibular, oculomotor systems Collaborative approach with GPs, specialists and the wider multi-disciplinary team Able to deliver individual community rehabilitation programmes focussed on independence and function Skilled in delivering home and community-based functional rehabilitation Ability to co-ordinate and collaborate with the wider multi-disciplinary team Experience working in inter-disciplinary teams to support people living with persistent pain Skilled in behaviour change, pacing strategies, and education in self-management Understanding of biopsychosocial approaches Skilled in functional needs assessments and service recommendations to support independence Strong understanding of ACC service entitlements and linking clients to appropriate supports Experience writing high-quality reports and justifying clinical reasoning Experience in musculoskeletal, neurological and vestibular rehabilitation (delete as appropriate) Experience in delivering gym-based, home-based, and functional rehabilitation programmes Skilled in assessing movement patterns and progressing strength and mobility Use your clinical expertise to support clients in homes, workplaces, and communities —no two days are the same Deliver care that aligns with our strong internal focus on excellence and outcomes Be part of a high-performing, supportive team that values collaboration, growth, and innovation A drive to make a difference and deliver outstanding care High-level clinical reasoning and adaptability across a spectrum of client needs A commitment to continuous learning and professional development Proven ability to deliver client-centred rehabilitation in community Clear, professional written and verbal communication with clients, whanau and stakeholders At least 2 years’ experience working within vocational rehabilitation and community rehabilitation services A competitive salary package 5 weeks of annual leave to support your work-life alignment A vehicle for business and personal use Entry level Full-time Health Care Provider Health and Human Services
